This course is designed to assist senior management staff, nursing leaders, case management personnel, and social workers in assessing their organization’s compliance with Medicare Conditions of Participation regarding discharge planning requirements. Nationwide there has been an increase in HCFA (or state) onsite visits to acute care facilities. Many of the visits have been prompted by complaints related to inadequate discharge planning.
